  •  Most current form of the scholarship application has been used.
  •  High school grade point average is listed and transcripts are attached.
  • Summary sheet from the high school is included, clearly communicating the academic scoring and/or grading scale used at that school.
  •  Scores from the ACT and/or SAT are listed with actual test results attached for verification.
  •  If the applicant has attended college, college grade point average is listed and college transcripts are attached.
  •  1040 tax forms for the previous year are included. Documented evidence of both parents' annual income is included, even in cases of divorce.
  • An essay written by the applicant is included, addressing all topics listed in the Essay section of the application.
  • Separate list of scholastic, ASHA, and community activities is included.
  •  Name, address and phone numbers of at least two references are included on the References section of the application, according to the recommended guidelines.
  •  Photo suitable for publication of the applicant is enclosed.

  • Must be in grade 11 or higher, age 21 and under, and be involved with American Saddlebred horses.
