  • Completed application form
  • A current photograph attached to the front of the completed application
  •  A current official transcript or, if applicable, an official transcript the last school attended
  •  An official report of SAT or ACT (or similar tests) scores, if a high school senior
  •  A resume with the start and end dates of all volunteer commitments
  • Two (2) written recommendation letters directly from the references

  • Applicants must be diagnosed with an inherited bleeding disorder. Students who do not have an inherited bleeding disorder but whose parent(s) or sibling(s) have an inherited bleeding disorder are not eligible to apply, even though they are affected by an inherited bleeding disorder.
  •  Applicants must have volunteered time and energy to directly benefit the bleeding disorders community.
  •  Applicants must be entering or attending an accredited 2 year or 4 year undergraduate institution in the United States. High school seniors, undergraduate freshmen, undergraduate sophomores, and undergraduate juniors may apply. Students who are pursuing or resuming an undergraduate degree later in adulthood may apply even if there was a gap in their academic career.
  • Applicants who are past recipients of the Beth Carew Memorial Scholarship may reapply throughout their undergraduate career.
  •  Applicants may not be a volunteer or staff member of the Colburn-Keenan Foundation or their immediate family members.
